Our clinic is open from 9 am to 5 pm Monday through Friday and the first Saturday of the month from 9 am to noon.
Sometimes the unexpected happens and you need to cancel your appointment make sure you call us as soon as you know you will not be able to make your visit. We allow up to three unexpected same day cancellations after which we will charge $25.00 if you do not show up for your appointment.
If you are experiencing an emergency you should call 911. For questions related to your care that needs more urgent attention, you can call our main number at (541) 735-3778 and you will be connected with Dr. Henderson or Dr. Kollmorgen right away.
First appointment, please bring the following:
- Insurance card
- A picture Photo
- A form of payment
- All current medications (or an updated prescription list)
Repeat Patient Appointments
- Any new medications (or an updated prescription list)
